To better provide a healthy, safe working environment, the Board of the North Judson-Wayne Township Public Library requests all staff members obtain the hepatitis B series of shots. The Library will pay for the series of three shots if obtained at the Starke County Health Department. Appointments for the shots can be made through the Health Department. A photocopy of the immunization card will be placed in the staff memberŐs personnel file.
If the staff member does not want to take the hepatitis B shot series, the waiver below must be signed for placement in the staff memberŐs personnel file.

WAIVER
I, ___________________________ decline to have the three shot series for Hepatitis B and will not hold the Library responsible for any illness, disease or health problem incured due to the lack of immunization for Hepatitis B.
