RIDER RESPONSIBILITIES

REFUSAL OF TRANSPORTATION

DRIVERS ARE NOT PERMITTED TO:

bulletTo be on time
bulletTo call at least one hour in advance
bulletTo not eat, drink, or litter in vehicle
bulletTo not use profanity
bulletTo wear your seatbelt
bulletTo treat driver and other riders with respect
bulletTo schedule rides which you intend to use
bulletTo pay your fare for each trip
bulletTo bring only service animals onboard
bulletto bring only as many bags as you can reasonable carry
bulletTo report any safety or security issues to the driver
bulletProvide 1 week notice for out of county trip

 

FAILURE to follow the above rules may cause you to lose the ability to schedule further rides

CSSC reserves the right to refuse service to any person for any purpose which may hamper our ability to provide service to our many patrons.

bulletUnsanitary potential health hazards
bulletInebriated, carrying open alcoholic beverages, explosives or weapons
bulletClient does not wear seatbelt
bulletClient's health and safety is in jeopardy
bulletInadequate vehicle or driver units to meet requests
bulletClients who frequently cancel scheduled rides or are "no shows"
bulletAll clients are expected to pay for each ride. CSSC reserves the right to restrict future rides for missed payments
"No Show" clients will have to call the office to verify any trips scheduled before we will dispatch a vehicle. We reserve the right to bill for "No Shows"

     CUSTOMER FEEDBACK

Customer feedback may be made by calling 772-7070. Customer compliments or complaints may be directed to the Transportation Manager or Executive Director. Complaints will be addressed following the grievance procedures. Request from Transportation Manager
 

SAFETY OR SECURITY ISSUES

Please report any suspicious activity to your driver immediately or contact dispatch at (574) 772-7070. Drivers will immediately notify management, who will be responsible for corrective action.
Accessible to the general public for trips to the doctor, dentist, senior service agencies, shopping, attorney, etc. from 8 am to 4 pm Monday thru Friday. Wheelchair lift available. 24 hour advance notice would be appreciated. The office will need to know the destination(s), the time you plan to leave your home and estimated time for returning home. Medicaid transportation provided. For information on fares, call Gail at 772-7070

Starke County TRIAD

bulletMeetings are held every quarter on the 4th Monday of the month at 12:30 pm  in July, October, January and April at the Community Services office.


bulletBeacon Flasher Lights: These are free to senior citizens who are on the Energy Assistance Program or to anyone with REMC electric service.  They may also be purchased for $15.00.  The adapter is inserted into your porch light and when you turn your light on and off a couple of times in a row, the porch light will flash continuously which will enable emergency personnel to find your home easier.


bulletFiles of Life:  The red magnetic sleeve contains a form on which you enter your name, address, physician, emergency contacts, prescriptions, past surgeries, allergies, diagnoses, religion and healthcare proxy information.  The File of Life should be placed on your refrigerator where emergency medical personnel can easily find it (they have been made aware that many seniors have these.)  There is no charge for the File of Life.


bulletPill Drops:  These are held twice a year to drop off outdated or unneeded prescriptions.  Please call the Community Services Office for dates and times (574-772-7070.)

Daily telephone check for individuals who live alone. Enroll yourself or someone you love. Referrals are made to Lifeline, an emergency response system

Senior Center Meals

Hot nutritious meals served five days a week to seniors 60 and over, at two locations. A minimum donation of $2.00 would be appreciated.
 
311 E. Culver Rd. Knox. The meal is served at 12:00 PM.
To make reservations call 772-6411 the day prior
Site is open 9:00 AM to 4:00 PM

North Judson Senior Center at 105 E Talmer Ave, North Judson. The meal is served at 12:00 PM.
To make reservations call 896-3665 the day prior
Site is open 9:00 AM to 1:00 PM for activities.

Home Delivered Meals

Home delivery of one hot meal five days per week, Monday thru Friday, to individuals who are unable to prepare nutritionally balanced meals. Clients must be assessed by the Area I case manager. 

Recipients are requested to contribute $2.00 per day to the cost of the meal. The meals are packaged at the Senior center and delivered by volunteers from 11 am to noon.  If you wish to volunteer by delivering the meals to the home bound, please call Patricia Minix at 574-772-7584.
